Wrath of the Black Manta follows a Ninja Warrior through a series of 5 Levels, as he attempts to track down "El Toro", and his gang who are behind a series of child kidnappings. Throughout each level the Black Manta may come across various clues to help him figure out where to go next or lead him to a hidden location.

In Japan, the game was originally titled Ninja Cop Saizou and had a slightly different plot and opening. Instead of a magnanimous ninja who decides to help his master locate his kidnapped pupil, Saizou is called by the police commissioner to pursue the case. The graphics and level design were also altered for the localization, with some stages moved around.

The game is unrelated to the DC Comics villain of the same name.

The Black Manta's arsenal of skills and weapons include: shuriken (throwing stars), "ninja arts" (which he obtains after completing a level), which allow Manta special powers, and abilities (such as Shadow, and Ground Fire to name a few). After each level he gains new abilities, which the player can then change / select from the menu screen.
